Medication Management:

Medication management is a level of outpatient treatment that involves the initial evaluation of the patient’s need for medications, the provision of a prescription, and ongoing medical monitoring related to the patient’s use of the medication by a qualified physician/prescriber.
Medication management includes monitoring, reconciling medications, and ensuring patients get the desired outcomes. It requires a thorough review of prescribed drugs and their possible side effects to create treatment plans and monitor their safety and efficacy.

Personalizaed Meal Management:
The combination of knowing what the client is going to eat, when they are going to eat it, and how how they are going to safely eat throughout the day. Meal management can be described as packing pre-cooked meal preps into meal management bags (specifically designed to carry multiple meals), that will be eaten at pre-scheduled intervals, in order to keep glucose levels steady, and energy levels high all day long. Basically meal management is the best process by which you are able to gain complete control over your clients dietary habits aloo=ng with dietary supervision.

Our onsite nurses are a source that is in place to promote health, safety, and wellness including:

Physical Therapy:
Physical therapy (PT) for many patients is an essential part of an interdisciplinary home health care team that helps individuals resume their daily activities and improve their overall health and wellbeing. It’s for patients that may be unable to leave the house to receive physical therapy, or may be more comfortable receiving PT services at home. Therapists and their assistants work with patients on mobility and lifestyle changes, monitor symptoms and also request interventions to help avoid unnecessary trips to the emergency room.
Physical therapy often helps to decrease the duration and severity of disabilities that arise from chronic diseases such as heart failure and COPD, and helps patients return to their desired daily activities, such as going to church or working in the garden.
